Bowel intussusception is rare in adults and usually driven by an underlying pathological process affecting the bowel. Renal cell carcinoma (RCC) most common type of kidney cancer its disease course, depending on initial histology stage, can metastasize to adrenal glands, lungs, bones, brains contralateral that be challenging follow. We present case a patient with history radical left nephrectomy for RCC developed acute bowel obstruction, secondary ileal metastasis RCC. In previous surgeries, small obstruction (SBO) found due adhesions, nonetheless high-grade at diagnosis, one must keep mind possibility relapse setting SBO.
